Programs
Basic Resource and Referral
Every client is screened for health insurance and either given referrals or assigned an appointment for enrollment assistance in a free or low cost insurance plan, if they qualify. Each client is called back to be sure they were able to access the referral that was given to them and to determine if they need additional resources or services.
Case Management
Those families who have multiple needs for resources or assistance accessing social services receive personal assistance from a case manger, including help setting up attainable goals.
Family Success Teams
Cased managed families who are very motivated are chosen to participate in this strength based approach to receiving resources, case plans with strict goals, and expectation of accountability ad tremendous support from a team of representatives fro local agencies such as the housing authority, child protective services, El Concilio, Family Resource & Referral, The Women’s Center, Crisis Pregnancy Center and San Joaquin County Behavioral Health (Tracy Center.)
Senior Safety Partnership
Free home safety inspections available to Tracy seniors. Those who need repairs and are unable to afford them are eligible for financial assistance fro the Tracy Hospital Foundation and free repairs from volunteers including local rotary club members.
Health Insurance Enrollment Assistance
Charterhouse Center for Families utilizes our “visiting agency” office to provide enrollment assistance to those who qualify for Medi-Cal, Healthy Families or Healthy Kids insurance programs.
ESL – Rosetta Stone
Many of our client’s primary language is not English. This creates difficulties with employment, education, role reversal with parents who use their children as interpreters and adults who are unable to advocate for themselves or their children. We now offer computer based ESL courses that individuals can take at their own pace. Beginnings to advanced courses are available.
Enhanced Cancer Support Services
A nurse navigator is available to assist local cancer patients and their families by providing case management, advocacy and emotional support.
PAPRX – Patient Assistance Program RX
A computer program that allows resource center staff to complete pharmaceutical company applications for free or low cost medications for low income patents how receive $5 per prescription (for a 3 month supply) to offset computer program charge and postal costs.
Mommy and Me Support Group
Social support and breastfeeding assistance for new mothers and babies. Three classes offered per week, including one in Spanish.
Play Groups
Classes held each week for children ages 0 to 2 and 3 to 5. Encourages learning through play and educates parents on developmental stages and activities for children that maximize learning and development.
P.R.I.C.E Parenting Classes
Parenting classes that are easy to understand, encourage parent interaction and use common sense strategies to teach parents how to create boundaries, set limits and teach children about the consequences of their actions. Optimal for parents of children ages 0 to 12, the classes are one day per week for six weeks.
